Tick all issues observed during the client's movement assessment.

Squat Pattern Test

FAIL if heels rise before parallel depth. FAIL if hip crease cannot reach knee height. FAIL if knees collapse inside foot line. FAIL if torso approaches horizontal.

Hip Hinge Test (RDL)

FAIL if lumbar flexion occurs. FAIL if hinge stops above knees without rounding. FAIL if hips cannot fully lock out.

Upper Body Push Test (Push-Up)

FAIL if <10 strict push-ups. FAIL if scapula lose control.

Upper Body Pull Test (Row Strength)

FAIL if row <70% of bench. FAIL if shoulder blades cannot retract.

Overhead Mobility

FAIL if arms cannot reach ears without rib flare.

Single Leg Stability

FAIL if unable to balance 10 seconds. FAIL if knee collapses inward.

Physio Screening Tests

FAIL if thigh cannot fully extend when opposite knee pulled to chest. FAIL if pain or restricted hip movement occurs. FAIL if hands cannot reach opposite scapula.

Shoulder Special Tests

FAIL indicates supraspinatus involvement. FAIL suggests subacromial impingement. FAIL suggests rotator cuff impingement.

Balance / Neuromuscular

FAIL if reach difference >4cm between sides.
